Consent Forms

The state of Minnesota requires consent forms to be signed prior to receiving treatment. For your convenience we provide all forms available for download. We encourage you to review the forms online or print and bring your completed forms to your appointment. 

Not able to print? No problem. We will provide you copies before your service. 

Note: for some medical conditions a doctors note is required. We advise you to review the forms before your appointment so you are prepared.
